Mt. Macrina Manor’s social service staff strives to make the transition from hospital or any other previous setting to our facility a smooth one for resident’s and their family members. The role of the social worker in a long term care facility is to enable each individual to function at the highest possible level of social and emotional wellness. We understand that the healthcare system can be difficult to navigate and that an admission to a healthcare can be emotional for all, we are here to help walk you through it.

Throughout the resident’s stay, our staff maintains close communication with the clients and their families to ensure that all needs are being addressed. The social service staff works in collaboration with other departments to develop a plan of care, and to determine how we can assist clients in returning to their previous living arrangements if able. This includes referrals to home health agencies, hospice services, DME providers, AAA, etc.
